The Coordinator works closely with clients, their families, caregivers, care managers, and healthcare professionals to ensure the provision of quality care services, in accordance with applicable regulations and guidelines.
This position requires strong organizational skills, great writing skills, effective communication, computer skills, and compassion.
The ideal candidate will have prior experience in the healthcare, medical education special education, or related fields.
This position is well-suited for new college grads or anyone pursuing a college degree.
Responsibilities Client Assessment and Care Planning:
Conduct comprehensive assessments of client's needs and develop individualized service plans.
Collaborate with clients, their families, care managers, and healthcare professionals to determine appropriate services and support.
Monitor and evaluate clients' progress and adjust service plans as necessary.
Service Coordination:
Coordinate and schedule services, including Respite, Community Habilitation, Caregiver/Advocacy Support, and community activities.
Maintain regular communication with clients, families, and service providers to ensure effective coordination and delivery of services.
Advocate for clients' needs and rights, ensuring they receive appropriate support and resources.
Collaborate with internal teams and external agencies to secure necessary resources and support for clients.
Documentation and Reporting:
Maintain accurate and up-to-date client records, including assessment reports, service plans, progress notes, timesheets, consent forms, and related documentation.
Ensure compliance with all regulatory requirements and documentation standards.
Prepare and submit timely reports on client progress, service utilization, and outcomes as required.
Quality Assurance:
Participate in quality improvement initiatives to enhance service delivery and client outcomes.
Conduct regular reviews and audits to ensure compliance with regulatory standards and best practices.
Identify areas for improvement and make recommendations to enhance the effectiveness of HCBS programs.
